We're all not as perfect as we think we are.
Ray and I have both given each other some bad catches. Nothing that hurt us but some that makes us look at each other like, "What's up with that?!?". But we climb together all the time so we're bound to have some not-so-perfect belays. It's just the percentages.

charlie, You have never missed a spot for me that I can remember. I pay very close attention to climbers and belayers as they tie in and rig the belay. Very few climbers check their belayers and I rarely see a belayer check to see that the climber has tied in properly or that his buckle is doubled back.
