I've had something like this before. Is it like someone stabbed you with a knife? I haven't had it for a couple years now, but it would happen randomly. There was no reason why I would get "stabbed," (ie. movement) it would just happen suddenly and without warning; and it hurt really bad!
I didn't do anything for it, I told my doctor and she had a name for it but pretty much said don't be too concerned. However, I'm no doctor - so maybe you should get it checked out.

doesn't feel like the normal reflux I have every day. and yes, I'm on that wonderfully addictive drug nexium 80mg a day. Been on it for years, just waiting for the brittle bones to come now.

Nexium does nothing for me. I'm on Aciphex. Great stuff, but I had to suffer through two other PPIs failure before my insurance would pay for it. You might want to see about an upper GI. That would rule out the hiatal hernia, but I think you probably know that. Yep, the bones are brittle, but I'm elderly! hee hee

I've had like 5 upper gi's in the last 10 years. And they told me I had the hiatal hernia back in '02 but then it seemed to go away... not sure if it actually went away or if I never had it...hard to tell with the shitty doctors these days.
I am concerned that nexium is supposed to cause brittle bones after being on it for 8+ years, since I'm pretty close to that mark now.
I'll have to check out that aciphex stuff. Is it cheaper than 5 bucks a pill?
