So, while I have never, ever heard of someone getting hurt from a bolt failing due to victory whips, I have seen and heard of many other ways to injure yourself sport climbing. Here are just a few:

Falling going to the frist bolt, not stick clipped.
Lead fall on lower angled rock.
Lead fall on steeper rock, bad belay.
Lead fall, hit ledge.
Lead fall, dropped by belayer (usually with gri-gri)
Dropped by belayer while lowering.
Anchor cleaning error.
Various pulls, sprains, tears, etc from actuall climbing.

Any others?

Amen.

Also, add ground fall while clipping 2nd or 3rd bolts.

oh, and I've heard that getting your pubes pinched in your harness and yanked out is pretty awful, but don't have experience there.
