My sister was a PA on one of the 'big 3' nightly news casts here in Chicago, and explained to one of the anchors between broadcasts why the writing in a piece was misrepresenting an important fact, and when he did the story on a later broadcast, he actually overrode the prompter! Freaking amazing!

i was listening to NPR the other day and they said "in other news a man was found dead today, foul play was suspected".
and that was it, not even a location.
