Supposedly "professionals"were consulted regarding the setup. It seems the problem was just the harness, or the way it was worn...I wonder how a professional rigger would have approached the same stunt?

Regardless of what kind of harness it was, she needed to clip into it. You can't fall out of a properly fitting waist harness by flipping upside down.
